Thoroughly beath yolks and sugar in a bowl. Slowly add 1/3 cup of the hot milk to the mixture, stirring all the while. Then, over low heat, slowly add yolk mixture back to pan and stir. Stir over low heat until mixture thickens and coats the back of the spoon.
Immediately pour through a mesh sieve and add chilled cream, rum, vanilla, and nutmeg.
At this time you can also add any additions like chopped pecans or tiny chunks of candied ginger, or bits of dried cranberries. Stir until mixed well.
Chill custard for at least two hours, then follow ice cream maker manufacturer's instructions on mixing the ice cream.
